Is there a minimum UCM hardware revision required to support the ETH03 module?
Will it work on an older series UCM card that has an ETH01?

Firstly the UCM has to be one that is flash upgradeable.
Then IMHO it just boils down to whether it fits.  I have on my test kit one of the first GEN flash UCM's that also had a 9-pin serial port on the baseboard.
Whilst Eth1 & 2 fitted quite nicely because they had a cut-out in the PCB, eth3 doesn't so I cant use.  
I'm quite sure if it's "flash-and-fits it'll work"!
I'm sure Cytech will confirm or shoot me down in flames thought!

Correct
You need a UCM that has flash ie min firmware 5.100
The ETH submodule can be plugged on top
